My name is Jay McElrath, and I’m proud to own and operate Caring Transitions of Macon, serving Macon, Gray, Bolingbroke, and surrounding communities across Middle Georgia.
I was born and raised in Macon, and this city is more than just home—it’s part of who I am. Over the past two decades, I built a career in the mortgage and home financing industry, helping families navigate important financial decisions with clarity and care. I later transitioned into operational leadership for a local business, where I strengthened my skills in client service, organization, and team management. At every step, I’ve been focused on people—understanding their needs and helping them find solutions.
But it was a deeply personal experience that led me to Caring Transitions.
In 2023, I took time away from my career to care for my mother during her final year battling ALS. Alongside the emotional toll of caregiving, I found myself facing the overwhelming responsibility of handling her estate. My mother was the longtime owner of The Village Shop, a well-known antique store in Macon, and sorting through a lifetime of memories, inventory, and decisions was both exhausting and emotional. I realized firsthand how valuable it would have been to have professional, compassionate guidance during that transition.
That experience lit a fire in me. I knew there had to be a better way to help families—especially seniors—navigate life’s most difficult changes. That’s why I brought Caring Transitions to the Middle Georgia area.
Today, we provide full-service solutions for downsizing, senior relocation, estate sales, and home clean-outs, offering the kind of support I wish my own family had during our journey. We exist to take the weight off families’ shoulders during moments of transition—offering guidance, resources, and empathy when it’s needed most.
I’m fortunate to be joined in this mission by my partner, Leslie Vito, who brings a strong background in customer service, marketing, and education. Together, we are committed to building a business rooted in trust, compassion, and community connection.
